This tool walks you through what specialists actually look for, organizes what you’re already noticing, and turns it into a clear, printable summary you can hand your doctor or your child’s school. You know your child. This helps you say it in a way they’ll act on.

This is not a diagnosis — and it can't be.

No questionnaire can tell you whether your child has autism, ADHD, dyslexia, or anything else. Only a qualified professional, with a full evaluation, can do that. What this tool does is help you organize your own observations and notice which patterns are worth exploring — so you walk into that appointment prepared, with your concerns in writing, instead of starting from scratch. The questions here are informed by the signs specialists genuinely look for (tools like the M‑CHAT‑R, CDC milestones, and the Vanderbilt scale), but this is a starting point, not a test, and not medical advice. Trust it to help you ask better questions — not to answer them.
